Utah's Medicare provider network is concentrated along the Wasatch Front, with facilities serving both urban communities and rural areas across the state. This directory lists 54 Medicare-certified skilled nursing and care facilities — from Salt Lake City, Provo, Ogden, St. George, and Logan. Each facility is Medicare-certified under Part A, meaning it meets CMS standards for skilled nursing and rehabilitation care.
Use this index to find a specific facility by name, or navigate to a provider detail page for coverage and location information.
A Medicare-certified facility has been inspected and approved by CMS to bill Medicare for covered services. For skilled nursing, Medicare Part A covers up to 100 days per benefit period after a qualifying hospital stay of at least 3 days.
Only facilities with Medicare certification are covered under Part A. Not every nursing home in Utah holds that certification. Always verify certification status and plan acceptance with the facility before choosing.
Medicare Advantage network coverage varies by carrier. Use the Utah plans page to compare carriers and verify in-network facility coverage before enrolling.
